Patients Rights and Responsibilities
The consent and patient rights document describes some details about your professional relationship with Hyde Park Counseling Professionals and their Therapist. It also describes how health information about you may be used, disclosed and your access to this information. Please review the documents thoroughly and carefully. Please discuss with your therapist anything you do not understand. Once you feel as though all of your questions have been answered to your satisfaction, sign your initials on the designated lines after each section to indicate you have read the section in its entirety and agree to it. You have the right to review this entire document before signing. The terms of this contract may change and if so, you may receive a revised copy.
